    Health Coordinator (LPN/RN)
    • Will consider a 20 hour per week (PT), or 40 hour per week (FT)
    • Experience in the field of addictions/mental health preferred

     

    Encumbent will provide assistance with admissions process, conduct evaluations, and provide individual and group counseling while maintaining caseload in fast-paced OASAS licensed outpatient chemical dependency clinic/outpatient rehabilitation program.  Will complete health assessments and provide recommendations.  Assist agency in obtaining physical examination records and educating patient population on current health issues and healthy behaviors. Function as part of a multi-disciplinary team. 

    Must be able to provide some evening coverage.
     
     

    Chemical Dependency Counselor

    Educational Requirements and Certification:

    ·         Formal alcoholism training and applicable counselor training, plus three-years full time work experience specifically in alcoholism/substance abuse treatment, which combine to provide eligibility for credentialing as a Credentialed Alcoholism and Substance Abuse Counselor (CASAC) (CASAC-T is acceptable).

     

    Experience:

    ·         A background of specific alcoholism and substance abuse training and experience is required.  ACBC is specifically seeking applicants to provide services for the re-entry population and co-occurring disorders population.   Must be able to work 2-3 evenings per week.

     

    Duties:

    ·        Under the direction and supervision of the Supervising Chemical Dependency Counselor, the Chemical Dependency Counselor will provide individual, group and family counseling to persons suffering from chemical abuse or chemical dependency and their family or significant others.  Caseload size and composition will be geared to the level of skill and training of the counselor.  Client problems of a medical, psychological, or social nature, related or unrelated to chemical dependency will be referred for consultation with the Supervising Counselor and may be referred to other professionals for treatment of these conditions depending upon the findings determined in the consultation.

    ·        The Chemical Dependency Counselor will be responsible for keeping records of this care as required by agency rules.

    ·        In addition, the Chemical Dependency Counselor will be expected to participate in agency staff meetings and to avail themselves of staff development opportunities that may be required by the agency.
